What did I do wrong?

Hi everyone. I made a buttermilk pie twice. But each time I made them they did not turn out. The filling was grainy and not smooth like I expected. Each time it puffed up alot, then sank. The flavor was good but the texture was off. What did I do wrong? Did I add too many eggs? too much melted butter. Did I bake it too long.
If you have the answer or can help, please let me know.
